touch typing faster

I have long been a hunt-and-peck wannabee touch typist, but never had the patience or discipline.
Finally, however, I have mastered the key locations, and can manage about 15 wpm. The same online test gave me 30 wpm for two-fingered typing, but I think if I push myself in real use, I may be able to manage about 50% more than that, so 45 wpm, without compromising accuracy. One test actually gave me 70 wpm, but when I type like that while working, I make quite a lot of errors.
This is obviously not fast enough for real work. 15 wpm is a lot slower in proportion to 45 wpm. I am forcing myself to touch type for a while every day though.
Does anyone have any tips for how to get to a useful speed? Or is it just a question of "practice makes perfect"?
It may be worth mentioning that I decided to learn dvorak. Apart from the mooted ergonomic benefits, I thought it could only be an advantage to have the letters in different places from what my keyboard says.
